Our program is dedicated to reducing loneliness and social isolation for individuals 65+, inspiring positive emotions and enriching lives. By fostering meaningful connections, we’re helping people rediscover purpose, joy, and healthier lifestyles in a welcoming community.
Empowering Change Through Social Prescribing
Social prescribing is transforming lives by connecting people with community-based services and activities that address the root causes of loneliness and poor health.
For individuals, it enhances mental wellbeing, reduces isolation, increases confidence and encourages healthy lifestyles.

Our Journey: A Success Story
Living Our Best Life started in 2019, inspired by groundbreaking models in the United Kingdom like Mendip Health Connections and the Bromley by Bow Centre. Thanks to funding provided by Equity Trustees, we introduced a sustainable social prescribing model that’s making a real impact across Victoria.
Today, Living Our Best Life continues to expand, spreading across Melbourne’s Inner East, the Upper Murray Region, and beyond. By creating safe, welcoming spaces to reconnect with others, individuals are joining activities that nurture growth, inspire confidence, and foster deeper connections within their communities.
A Transformational Impact
For participants Living Our Best Life has reduced social isolation, built confidence, created safe and welcoming spaces to reconnect with others. Through a tailored, person-centred approach, individuals have been able to access services, develop new skills, and engage more fully in community life contributing to improved social health and wellbeing.
